Membership for EFWO and Benefit Chart "Z"

 All Employees are requested to coordinate for filling up Following Type of Membership Form
Form "A" for All Employees with Consent Form, Form "B" Only for Pensioners,  Form "C" for BPS-1 to 5, Form "D" for BPS- 6 to 10, Form "E" for BPS-11 to 16,  Form "F" for BPS-17 to 20/22.
Every Member should pay Rs.365/- one Time in Cash, EFWO will try to approve Code for deduction through Salary like BF& GI .
